Kidney risk assessment

Kidney health is more than a creatinine number.

Kidney risk can be influenced by several factors. A structured assessment looks at the bigger clinical picture rather than a single laboratory value.

  1. Assess
  2. Stratify
  3. Manage
  4. Monitor
  5. Adjust

When diabetes and kidney disease meet, management must be connected.

  • Diabetes management
  • Blood pressure management
  • Kidney-function monitoring
  • Proteinuria assessment
  • Metabolic-risk management
  • Medication review
  • Specialist review
  • Longitudinal monitoring

How risk factors can add up

  1. Diabetes
  2. Blood pressure
  3. Protein leakage
  4. Kidney stress
  5. Declining function

Assess → Manage → Monitor → Adjust

Risk factors can interact over time, though not every patient follows this exact sequence. Early assessment and structured management may help address modifiable risks.

Blood pressure matters to your kidneysPersistent high pressure can add stress to the kidneys, so it is built into the overall kidney-care strategy.Pressure → Kidney stress → Risk assessment → Management → Monitoring
Risk-factor focusBlood sugar, blood pressure, protein leakage, metabolic health, cardiovascular risk and medicines are reviewed together.
Help delay progressionWhere clinically appropriate, the aim is to manage risk factors and help delay disease progression.
Advanced clinical management

Structured, individualised, specialist-guided.

Where clinically appropriate, care follows individual assessment, patient selection, specialist recommendation and ongoing monitoring.

Risk-factor optimisationStructured management of diabetes, blood pressure, metabolic health and other modifiable kidney-risk factors.
Specialist-guided interventionsWhere clinically appropriate, patients may be considered for advanced clinical interventions based on assessment, suitability and specialist recommendation.
Monitoring & adjustmentClinical response, kidney function, blood pressure, metabolic parameters and relevant investigations are reviewed to guide ongoing care.

Specific treatment and intervention decisions are individualised following clinical evaluation.

Frequently asked questions

What does protein in the urine mean for someone with diabetes?

Protein or albumin in the urine can be one sign that the kidneys are under stress. It needs to be interpreted with other results, such as eGFR and blood pressure, and does not confirm a diagnosis on its own.

What are creatinine and eGFR?

Creatinine is a blood test marker. eGFR is an estimate of kidney filtering function calculated from it. Clinicians use both, with urine tests, to assess kidney health over time.

Can kidney disease progression be slowed?

In suitable patients, specialist-guided management of blood sugar, blood pressure and other risk factors may help slow progression. Outcomes vary and cannot be guaranteed.

Does Boston Renal provide dialysis?

No. Boston Renal is a day-care clinical management centre and does not provide on-site dialysis. Patients who need dialysis or hospital-based care are referred or coordinated with the relevant specialist or hospital.

What should I bring to my first assessment?

Recent kidney function and urine test reports, HbA1c or blood sugar records, blood pressure readings, and a list of current medicines. If you do not have everything, you can still book.
